The nursing breast
It takes around two to three days after the birth before breast milk starts to flow. The breast initially produces colostrum, which is particularly rich in nutrients and antibodies. Around the third day the breast starts to fill with milk and "lets down".
Milk production is triggered by breastfeeding hormones. They ensure that milk is produced in the lactiferous glands which then flow through the milk ducts to the nipple during nursing.
The breast is often hard and full when the milk is let down and fills again for the first time. The breast is also filled with lymphatic fluid and its blood circulation increases at this time. These changes can be quite painful and leave the breast and nipple feeling very sensitive. However, these initial difficulties are soon overcome, especially with the help and support of a breastfeeding counsellor.
